Job Description:
**LPN Medication Care Manager Job Description**
Provide high-quality care and services to seniors by administering
medication and treatments in a safe and organized manner.
**Job Responsibilities**
As a member of the Sunrise team, you will support our mission to
champion the quality of life for all seniors. Your responsibilities
will include:
* Administering medication and treatments to residents in a safe
and organized manner
* Documenting medication administration and resident care
* Conducting Service and Health Updates as directed by the Resident
Care Director
* Participating in the development of Individualized Service Plans
(ISPs) and monthly updates
* Transcribing orders
* Maintaining confidentiality of resident information
* Reporting resident concerns and accidents/incidents
immediately
* Ensuring a safe and clean environment for residents
* Participating in training and contributing to team success
**Medication Administration**
* Administer medications and treatments to residents using the
medication administration record and the Six Rights of Medication
Pass
* Review and document medication updates from the Resident Care
Director or Wellness Nurse
* Ensure medications are passed according to times and document any
refusals or missed doses
* Maintain confidentiality of resident medication information
* Report any resident concerns made while administering medication
to the Resident Care Director or Wellness Nurse
**Resident Care**
* Notify the Resident Care Director of any resident and/or family
concerns through the LEAD process
* Attend and actively participate in daily Cross Over meetings
* Conduct Service and Health Updates as directed by the Resident
Care Director
* Participate in the development of Individualized Service Plans
(ISPs) and monthly updates
* Transcribe orders
**Risk Management and General Safety**
* Partner with the community team to ensure compliance with OSHA
and Occupational Health and Safety Regulation requirements
* Practice safety procedures at all times, including Personal
Protective Equipment (PPE) and fire extinguishers
* Report all accidents/incidents immediately
* Report all unsafe and hazardous conditions/equipment
immediately
* Ensure a safe and clean environment for residents
**Training and Contributing to Team Success**
* Actively participate as a member of a team and commit to working
toward team goals
* Demonstrate the Team Member Credo in daily interactions with
others
* Commit to serving residents and guests through our Principles of
Service
* Contribute to the overall engagement programs and processes
* Attend regular meetings and training sessions
**Qualifications**
* LPN, LVN, or state/province specific licensed nurse
credential
* Maintain certifications and ongoing training and re-education as
required by Sunrise and state/provincial regulations
* Must be at least 18 years of age
* Previous experience working with seniors preferred
* Desire to serve and care for seniors
* Ability to make choices and decisions and act in the resident's
best interest
* Ability to react and remain calm in difficult situations
* Ability to handle multiple priorities
* Possess written and verbal skills for effective communication and
level of understanding
* Demonstrates good judgment, problem solving and decision-making
skills
