The Licensed Practical Nurse (LPN) position coordinates total plan of care with other health care professionals involved in care, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. Essential FunctionsProvides supervision and oversight to patient care as outlined by State regulation and/or requested by Agency Director.Utilize the E.V.V. (Electronic Visit Verification) program as applicable.Assist in the orientation of new staff as needed.Prepares clinical notes daily.Reports to the Agency Director or Designee on a regular basis regarding the general, physical and mental condition of the patients.Administers prescribed medication and treatment as permitted by state or local regulations and in accordance with the physician's orders.Prepares physicians orders as applicable.Participates in in service education programs, patient care conferences and staff meetings as scheduled.Follows company policies and procedures.All other duties as assigned. Education & ExperienceExperience: 1 year License, Registration, and / or Certification Requirement: Yes Education Requirements Must meet educational/experience requirements outlined in the laws of licensure states. Experience Requirements A minimum of one year of acute care experience required; previous home care experience preferred. License Requirements Must have current LPN licensure in state of practice. Current drivers license, vehicle insurance and access to a dependable vehicle or public transportation. Current CPR certification required. Skill Requirements Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems. Must possess strong, patient management, and documentation skills. Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team. Must possess a strong commitment to the goals, mission, and philosophy of the organization. Ability to work flexible hours as required to meet identified patient needs. Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments. Ability to adapt to changing organizational needs.
