Get ready to start doing your life's best work.SM In this Health and Social Services Coordinator role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Our team is made up of RNs, Social Workers, Behavioral Health Specialists, and others dedicated to helping members achieve their health care goals.This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of (8:00am - 5:00pm Central Standard Time). It may be necessary, given the business need, to work occasional overtime. Our work site is located at Our Lady of the Lake Medical Center 5000 Hennessy Blvd. Baton Rouge, LA 70808; position will be 75% located at the hospital and 25% remotely.*All Telecommuters will be required to adhere to UnitedHealth Group's Telecommuter PolicyPrimary Responsibilities:

Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of careIdentify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive servicesManage the care plan throughout the continuum of care as a single point of contactCommunicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all membersAdvocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:
High School Diploma / GED (or higher) - Current RN, LMSW, LCSW, or LBSW unrestricted license or similar in the State of LA 4+ years of experience working within the community health setting or in a health care roleIntermediate Microsoft Office experience working with Microsoft Word, Excel, and Outlook (ability to create, edit, save, and send documents, spreadsheets, and emails)Ability to travel up to 10% of the time in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, nursing facilities or providers' officesAbility to work an 8 hour shift between 8:00am - 5:00pm CST including the flexibility to work occasional overtime per business needFull COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.
