Client Assessment SpecialistAbout the RoleThe Client Assessment Specialist offers an exciting opportunity to positively impact your career and community!ResponsibilitiesConducting in-home assessments for LT-PCS applicants and participantsProviding HCBS program choice counseling to requestorsProviding an overview of the Long-Term Care Program to applicants (Rights and Responsibilities)Developing a needs-based and Person-Centered Plan of CareCompleting client monitoring surveys as part of quality managementRequirementsPossess Bachelor's degree in Health/Human Services field, Social Work, or related discipline, or Registered Nurse DegreeDisplay 2 years of health related social and/or human services experienceShow a minimum of 1 year experience working with older adults or persons with disabilitiesMust live in LouisianaMust possess a valid driver's licenseFlexible WorkingAt Conduent, we want you to be yourself.
